O Fount of Life, unfathomable Divine Mercy

I’ve been listening to a relatively new radio program by St. Michael’s Media, called “In Your Faith Radio.” The host, Michael Voris, opens the program with a bit of an optional prayer that’s used at the beginning of the Chaplet of Divine Mercy (my favorite private devotion). Here’s the full prayer:
“You expired, Jesus, but the source of life gushed forth for souls, and the ocean of mercy opened up for the whole world. O Fount of Life, unfathomable Divine Mercy, envelop the whole world and empty Yourself out upon us. O Blood and Water, which gushed forth from the Heart of Jesus as a fountain of Mercy for us, I trust in You!”
I wonder why this profound and beautiful prayer wasn’t included in my Divine Mercy prayer book. I had to search the ‘net to find it.

My first guess is that it’s in the Diary of St. Faustina but was overlooked in the prayer book (or cut out because there were so many excellent passages).
My second guess is that perhaps it’s from Sister Josefa Menendez, who apparently received similar revelations as Sister Faustina. (I haven’t read much of that yet – my sponsor pointed me to it during Holy Week and I’ve ordered the booklet.)
